Case Management
Case management involves situations where we are called upon to extend our care beyond a single assessment and intervention. When assigned to manage a case, we take care of all aspects pertaining to an individual’s rehabilitation or development journey over an extended period of time.
This extended responsibility implies that we engage and work with a number of stakeholders, including family members, caregivers and other professionals.
We ensure that the needs of the individual and those providing care are understood and aligned through a well-thought-through case plan, which we revise as and when the need for adjustment is recognised.
